What the form is, and what DEEP said about it

What DEEP requires
Every certified commercial supervisor files the Commercial Applicator Pesticide Use Summary Report by January 31 for the prior year: product name, EPA registration number, and total gallons or pounds. Certification renews every five years with 12 credits per category and all five years of summaries on file.
What DEEP told us in writing (September 11, 2026)
Parts I, II and IV of the form must stay as they are. Part III may be presented as your own table as long as it shows product name, EPA registration number, and total amount before diluting. That is exactly what the packet produces.

What this is, and isn't
- It prepares a draft for your review. You remain the one who signs and submits.
